Your dedicated case manager will spend time getting to know you to truly understand your needs. They will work with you to develop your own specialised plan and provide you with the best possible care.

“The work I do as a case manager I find rewarding, working closely with the client and their family to determinewhat’s best in terms of supports and services to help them daily and keep them comfortable and independent in their own home for as long as possible. We’re also able to alleviate stress on their family carers as well,” said Anastasia from the aged care and disability support team

Most of the IPC team come from multicultural backgrounds and speak languages in addition to English. We can also arrange free interpreters.

IPC Health’s aged care services team works with people aged over 65 years and Aboriginal and Torres Strait Islander people 50 years and over, as well as people experiencing or at risk of homelessness in Victoria.
