Would you like help with day-to-day tasks to make sure you stay in your own home for longer? IPC Health can support you to apply for and manage your Home Care Package, connect you with the services you need and more!

Getting the right level of support can be a daunting process so we’re making sure that our aged care and disability support services are easy to access and understand. Our team offers in-home visits with guidance and practical help, from the application stage and setting up appropriate services to tailored care management and regular reviews to make sure everything is running smoothly.

Your personalised care plan may include:

• personal carelike showering and getting dressed

• help around the house cooking, cleaning, gardening or changing a light globe

• shopping assistance

• transport to get to and from appointments

• nursing

• allied health care such as podiatry, dietetics, physiotherapy, or counselling to ensure you are safe and well in your own home

IPC Health is an approved provider of Home Care Packages and we can help you access a range of services so you can stay living in your own home for longer with all the supports that you need to live safely and independently. Your dedicated case manager will spend time getting to know you to truly understand your needs. They will work with you to develop your own specialised plan and provide you with the best possible care.

“The work I do as a case manager I find rewarding, working closely with the client and their family to determinewhat’s best in terms of supports and services to help them daily and keep them comfortable and independent in their own home for as long as possible. We’re also able to alleviate stress on their family carers as well,” said Anastasia from the aged care and disability support team

Most of the IPC team come from multicultural backgrounds and speak languages in addition to English. We can also arrange free interpreters.

IPC Health’s aged care services team works with people aged over 65 years and Aboriginal and Torres Strait Islander people 50 years and over, as well as people experiencing or at risk of homelessness in Victoria. For social work support, we help older people and their carers as well as people of all ages with disability and their carers living in Brimbank, Hobsons Bay and Wyndham.
